Keeping Life in Motion.

Welcome to Active Health Collective

Whether you’re visiting Active Health Collective for the first time or you’ve previously known us as Active Chiropractic, we’re glad you’re here.

Our Edmonton clinic offers chiropractic care, massage therapy, kinesiology and rehabilitation, shockwave therapy, spinal decompression, dry needling/IMS, therapeutic laser treatment, and custom orthotics.

Your first appointment will vary depending on the service and practitioner you’re seeing, but we’ve gathered the information below to help you know what to expect and prepare for your visit.

Your First Appointment

What to Expect

Your first appointment is an opportunity for your practitioner to learn more about you, understand what brought you to the clinic, and determine an appropriate approach to your care.

Depending on the service you’ve booked, your appointment may include:

  • A discussion of your current concern and goals
  • Review of relevant health and medical history
  • An assessment or examination
  • Discussion of clinical findings, where applicable
  • An explanation of available treatment options
  • An opportunity to ask questions
  • Treatment, when appropriate

The exact structure of your appointment will depend on the practitioner and service you are seeing.

Your practitioner will explain what they recommend and obtain appropriate consent before proceeding with treatment.

Patient Forms

Complete Your Forms Before Your Appointment

New patients may be asked to complete intake and consent forms before their first appointment.
Completing your forms ahead of time gives your practitioner important information about your health history and the reason for your visit while allowing you to spend less time on paperwork when you arrive.

If you’ve already completed the appropriate forms through our online booking system, you do not need to complete them again unless instructed by the clinic.

What Should I Bring?

What you’ll need can vary depending on your appointment, but it may be helpful to bring:

  • Your extended health benefits information if you plan to use insurance
  • A list of relevant medications or health information
  • Any relevant reports or imaging information available to you
  • A referral or prescription if required by your benefits provider
  • Comfortable clothing appropriate for movement or assessment, depending on the service

If you’re unsure whether you need to bring something specific, contact the clinic before your appointment.

What Should I Wear?

Comfortable clothing is a good choice for many appointments, particularly when movement or physical assessment may be involved.

For chiropractic, kinesiology, or rehabilitation appointments, clothing that allows you to move comfortably can be helpful.

For massage therapy, dry needling/IMS, or other treatments, your practitioner will explain what access is required for the area being treated. You will only be asked to adjust or remove clothing as appropriate for the treatment and within your comfort level.

Direct Billing & Extended Health Benefits

Active Health Collective direct bills all major insurance companies.

Many health benefit plans provide coverage for services such as chiropractic care, and massage therapy. Coverage for other services can depend on the practitioner providing the treatment and the terms of your individual plan.

Insurance plans vary, so we recommend checking your individual policy if you have questions about:

  • Coverage amounts
  • Referral or prescription requirements
  • Practitioner eligibility
  • Annual limits
  • Custom orthotics requirements
  • Documentation required for reimbursement

Coverage is determined by your benefits provider and individual plan.

Frequently Asked Questions for New Patients

Your first chiropractic visit begins with a consultation to discuss your health history, current concerns, and what brought you to the clinic. Your chiropractor will then perform an examination, which may include specific tests to help assess and diagnose your concern.

If diagnostic imaging is appropriate, your chiropractor can refer you for X-rays or other imaging such as CT scans, MRI, bone scans, or bone densitometry when warranted.

Once your history, examination, and any relevant test results have been reviewed, your chiropractor will discuss the findings, diagnosis, and recommended next steps with you. If chiropractic treatment is appropriate, treatment may include chiropractic adjustments, soft tissue techniques, physical therapies, or other techniques selected according to your individual needs.

You may also receive recommendations for home care, such as exercises, stretches, or modifications to certain activities.

Most services at Active Health Collective can be booked directly. However, your extended health benefits provider may have its own referral or prescription requirements for reimbursement. Check your individual plan if you’re unsure.
Completing any required intake forms before your appointment can help reduce the amount of paperwork needed when you arrive, approximately 10 minutes early.
